All boxes are required to submit your application.

I understand that submitting an application does not guarantee assignments or hours. I understand that I choose which opportunities I accept. I agree to respond to assignment offers whenever reasonably possible with ACCEPT or PASS. I understand that once I accept an assignment, the family and Lillie's Leaping Learners are relying on me to complete it. I understand that repeated no-responses, avoidable last-minute cancellations, or no-shows may affect my Active Provider status. I understand that approval for one service category does not automatically approve me for another. I understand that additional documentation or screening may be required depending on the type of assignment.

Background check consent — I understand that Lillies Leaping Learners requires a comprehensive background check as part of provider onboarding, and I consent to this check being conducted if my application moves forward.
